Ubicado en el tren Oriente Express en 1914, el jugador asume el papel de Robert Cath, un médico estadounidense en el último viaje del tren de París a Constantinopla justo antes de la Primera Guerra Mundial. Tras el asesinato de un policía irlandés, Cath es buscado por la policía francesa. Él se pone en contacto con su viejo amigo, Tyler Whitney y planean reunirse en el Oriente Express y poder ayudar a Cath a escapar.
